Splendid membership plan is not insurance and it is not a qualified health plan under Affordable Care Act.
- Membership fee is due at time of enrollment.
- Plan duration is one (1) year from sign-up date.
- Cannot be combined with any other offers or dental insurance.
- Unused services within the plan duration do not carry over.
- Not transferable to another party.
- Membership fee is not refundable after 30 days, or if any benefits have been used.
- The benefits of this plan are limited to procedures completed only at Splendid Smiles Dental Studio, 571 Frost Ave, Warrenton VA 20186.
- The benefits of the plan can not be applied to treatment already in progress.
